Welcome to Balancing Health Medical Center. Please read these Terms and Conditions (“Terms”) carefully before using our website, bhmcnd.com, or enrolling in our medical programs. By accessing or using our services, you agree to be bound by these Terms.
1. Legal Entity & Management Disclosure
The website, branding, and administrative infrastructure are owned and managed by Balancing Health Medical Center, LLC (a Wyoming entity). Medical, telehealth, and clinical services are provided by Balanced Health Medical Center, PLLC, an Arizona professional limited liability company doing business as Balancing Health Medical Center. All clinical decisions, medical protocols, and patient care are under the exclusive direction of licensed healthcare professionals.
2. Eligibility & Geographic Restrictions (UPDATED)
To enroll in our clinical programs or utilize our telehealth platform, you must be at least 18 years of age. Medical and telehealth services are strictly limited to individuals physically located within 48 U.S. States and the District of Columbia. Services are unavailable to residents of Alabama and Mississippi. By initiating an intake form or transaction, you attest that you are physically present in a permitted jurisdiction at the time of your clinical consultation.
3. Telehealth Framework & Independent Provider Network (UPDATED)
- No Emergency Services: This platform is not an emergency response system. If you are experiencing a medical emergency, call 911 or visit the nearest emergency room immediately.
- Network Integration: To provide seamless, nationwide clinical coverage across 48 states, Balanced Health Medical Center, PLLC partners with Qualiphy, an independent, secure, third-party telehealth clinician network.
- Independent Medical Discretion: Financial submission or intake completion does not guarantee a prescription. Prescriptions and Patient-Specific Orders (PSOs) are issued strictly at the independent professional discretion of the state-licensed, credentialed advanced practice provider assigned to your chart via the Qualiphy network and our secure OptiMantra Electronic Health Record (EHR) system. If you are deemed clinically ineligible for treatment following your medical chart review, a full refund will be processed minus a non-refundable administrative consultation fee.
4. Subscription Programs & Billing Transparency
By enrolling in our medical programs, you authorize Balancing Health Medical Center to charge your payment method on file automatically every month.
- Testosterone Replacement Therapy (TRT) Program: Subscriptions start at $250.00 per 4 weeks. This covers ongoing medical oversight, provider consultations, and your prescribed medication therapy, subject to clinical approval.
- GLP-1 Weight Loss Program: Subscriptions start at $375.00 per month. This covers ongoing medical oversight, provider adjustments, and your prescribed GLP-1 weight loss medication, subject to clinical approval.
- Price Modifications: Price points reflect baseline starting doses. Titration, dosage increases, or specialized medication choices may alter monthly subscription pricing. You will always be notified of pricing adjustments before being charged.3

5. Cancellation & Refund Policy
To maintain compliance with merchant processing guidelines and ensure seamless clinic operations, we enforce a strict billing cycle policy:
- Cancellation Procedure: You may cancel your monthly medical subscription at any time. To prevent your card from being automatically processed for the next month, you must submit a written cancellation request via email to info@bhmcnd.com at least five (5) business days prior to your scheduled billing renewal date.
- Refund Policy: Due to the customized nature of prescription medical therapies and clinical evaluation services, all sales are final. No refunds will be issued once a medical consultation has been performed or a prescription has been sent to the pharmacy.
6. Privacy & HIPAA Compliance
Your use of this website is also governed by our Privacy Policy. While administrative and website support is managed by Balancing Health Medical Center, LLC, all protected health information (PHI) is handled exclusively by Balanced Health Medical Center, PLLC in strict compliance with HIPAA standards.
7. SMS/Text Messaging Program Terms
By opting in or submitting a form on bhmcnd.com, you consent to receive recurring automated text messages (SMS and MMS) from Balancing Health Medical Center, including care coordination, practice announcements, and continuity of care updates.
- Opt-Out: You can cancel the SMS service at any time. Just text “STOP” to our number. After you send the SMS message “STOP” to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time.
- Help: If you are experiencing issues with the messaging program you can reply with the keyword “HELP” for more assistance.
- Interruption: Carriers are not liable for delayed or undelivered messages.
- Cost: As always, message and data rates may apply for any messages sent to you from us and to us from you. Message frequency varies based on your interactions with our practice. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
